Job Title:   FAMILY NAVIGATOR-

Service area: Northeast Tennessee and Southwest Virginia

Responsible to:    Senior Project Manager

 Job Purpose:   Our Family Navigators are skilled in identifying individual family needs and resourceful in locating and connecting them to appropriate education, care, and community resources. From personal, place-based case management to agency-wide program development, this full-time, grant-funded position provides you with the opportunity to assist our community in a real and impactful way.

Primary Duties and Responsibilities

Our clients and staff alike come from all walks of life. The role of Family Navigator is open to a wide variety of backgrounds and experiences. Required qualifications include:

  • BS. /B.A. degree in Human Services, Social Work, or related field.
  • Experience in case management, including comprehensive knowledge of all applicable rules, regulations, and industry standards.
  • Proficient in using a variety of computer software applications, as they apply to the position.
  • Dependable, strong work ethic.
  • Ability to define problems and to create and implement solutions.
  • Ability to work independently and as a team.

Case Management

Much of your time as a Family Navigator is allotted to case management. You will provide personal, ongoing, non-judgmental, and empathetic support, services, and education by:

  • Assisting families in developing and implementing Personal Plans.
  • Meeting weekly, and as needed, with families to achieve their Personal Plan goals.
  • Linking families with other agencies and services; accompany participants to meetings and appointments as needed/requested.
  • Compensating for rural travel and geographic barriers by traveling to meet with clients in a confidential and safe location as needed and schedule availability allows (community centers, libraries, schools, etc.).
  • Managing grant requirements and reporting standards.
  • Communicating effectively with persons of diverse socioeconomic and cultural backgrounds.

Program Development

The other key element of the Family Navigator position is program development. You will work both independently and with a team to develop, implement, deliver, and assess both in-person and virtual activities to support families by:

  • Developing and maintaining a web-based resource library that directly meets clients’ needs.
  • Networking with partner agencies to identify families’ unmet needs and to developing and/or enhancing the services/options available.
  • Developing, overseeing, and maintaining an evaluation tool to process and access the effectiveness of services in compliance with program objectives.
  • Facilitating a client satisfaction survey to solicit feedback for the advancement of policies, procedures, and future programs.
  • Completing narrative and data reporting to support grant objectives using the organization’s case management software.

General

 Job Title:     TEEN PREGNANCY PROGRAM COORDINATOR-

Responsible to:  Director of Teen Pregnancy Programming 

Job Purpose:    The Teen Pregnancy Program Coordinator is responsible for the overall management, coordination, daily operations and service implementation and delivery of the teen pregnancy education and support program.

Primary Duties and Responsibilities

The MOMS R US Program Coordinator performs all of the following: 

Teen Pregnancy Program Management

  • Deliver a comprehensive teen maternal and fetal health initiative
  • Collaborate with community resources (e.g., school systems, health department, social services, etc.) to ensure the service availability for pregnant and parenting teens.
  • Active participation in community groups and committees and committees designed to address infant, youth and teen health and human services.
  • Network with other community resources and agencies to identify any unmet needs for teen parents in the service area.
  • Develop and implement innovative services that address the special needs of pregnant and parenting teens from diverse and under-served populations.
  • Maintain evaluation metrics to access the effectiveness of service and compliance with program objectives and grant requirements.
  • Obtain client satisfaction surveys, along with other methods of soliciting feedback to incorporate into policies, procedures and program development.
  • Assist in preparing the narrative and data reporting to support grant objectives. 

Advocacy and Case Management

  • Provide ongoing support, services and education to teen parents
  • Conduct initial intake with teen parents entering the teen pregnancy program.
  • Provide the trauma informed care approach with non-judgmental, empathetic, supportive advocacy and education for teen parents
  • Provide comprehensive follow-up contact and services.
  • Provide regularly scheduled educational classes and parenting mentorship specific for teen parents.
  • Facilitate ongoing teen parenting mentorship and support groups at various locations throughout the community
  • Assist, support and offer one-on-one case management services to program participants based on unique needs of teen parents
  • Engage other health and human service agencies on behalf of participants based on unique needs of teen parents.
  • Provide logistical support for teens to attend prenatal appointments as well as follow-up appointments for the teen and infant.
  • Provide logistical and material support for teens applying for human or social service benefits (e.g., WIC, SNAP, Childcare Assistance).
  • Provide logistical and material support for teens navigating the justice system 

Education and Outreach

  • Provide information to local health, human services, municipal facilities and other community agencies addressing services for pregnant or parenting teens.
  • Provide teen pregnancy prevention presentations for community agencies (as requested).
  • Provide, facilitate and promote Trauma Informed Care trainings for agencies serving teens.
  • Conduct informational presentations for high schools, institutions of higher learning and other community agencies on teen pregnancy and the unique challenges of teen parenting.
  • Implement and maintain an ongoing schedule that provides information and community outreach about teen pregnancy awareness throughout service areas using printed materials, media and educational events.

General Duties and Responsibilities

Administrative Functions

  • Ensures YWCA Hallmark standards of racial justice initiatives and economic development for women are components of all major YWCA programs
  • Functions as staff associate to board committees when assigned
  • Participates in conferences, training sessions, and meetings relating to areas of assigned responsibility
  • Responsible for ensuring compliance with all YWCA policies and guidelines within area of responsibility
  • Performs other duties as assigned by the Director of Teen Pregnancy Programming, Chief of Staff of Chief Executive Officer

Responsible to:           CEO

 Job Purpose:   The Senior Executive Assistant is responsible to support our CEO in a timely and professional manner. Position provides high-quality administrative and clerical assistance. The Senior Executive Assistant’s main duties include creating correspondence, organizing daily schedules, managing donor cultivation and meetings. To be successful in this role, you should be proactive, meet deadlines and communicate effectively. Our ideal candidate also has previous experience as an executive assistant and is familiar with office management technologies. Ultimately, you should be able to identify and address the needs of the CEO and perform administrative tasks to ensure workflow runs smoothly.

Primary Duties and Responsibilities

The Senior Executive Assistant performs the following:

Office Management and Administrative Duties

  • Prepares notices, materials and set up for Board of Directors and Administrative Staff Meetings
  • Schedules appointments and provides information to callers and staff
  • Oversees and carries out various day-to-day departmental administrative functions
  • Composes correspondence and reports
  • Conducts internet research and collects data for reports and meetings
  • Compiles a variety of information from both manual and computerized records and files for regular and special reports and in response to specific requests

  • Assists with employee incentive programs as a means for team building
  • Performs the duties of Staff Liaison for volunteers and volunteer projects
  • Performs basic liaison functions with other departments / programs
